Annual Review

Once again, this year's annual review is being brought to you on a day and at a time that works best with your schedule.  One of our team members will be working with you to arrange a day within the window that starts September 15, 2011 and ends on November 15, 2011.  By allowing the review process to take place locally through subcommittees in your area, the following benefits will occur:

  • More personal and planter-focused – The time with the subcommittee will be concentrated on you and your church, rather than be broken up into a variety of segments on a tight schedule.  This will allow the review to be more relaxed and less time-constrained.
  • More informative - The review will take place in your community and potentially at your facility.  It will be much easier for you to share some of the good things that God is doing and convey some of the challenges you may be facing.
  • More convenient – You can work with the committee members to schedule the day and time that works best for everyone.  Also, transportation is localized and much easier.
  • More aligned with the NA system – Network Affiliated churches (formerly known as District Affiliated churches) have a localized review system.  Therefore, planters will have greater continuity in their review method as they transition from planting status to Network Affiliated status.
  • Less expensive – The overall expenses for the review can be reduced providing more financial resources to plant churches.

Below (at the very bottom of the page) you will find the forms and worksheets to be used for annual review.  Please complete the applicable forms and mail them c/o Amber Jones, Ohio Ministry Network, 8405 Pulsar Place, Columbus, OH 43240 by September 9, 2011.  If you have any questions or concerns, please do not hesitate to contact the OCMN office at 614-396-0700.

Please take note of the following instructions when completing the attached documents:

PastorAnnualReport.doc - Please complete this report as thoroughly as possible in order to help maximize the review experience.  The focus is the pastor's personal and family health.

AdvisoryCommitteeForm.doc - This document is to be given to prospective advisory committee members.  It allows the pastor to gather information when considering potential advisory committee members and is ultimately used by the network superintendent to approve advisory committee appointments.  Please note that one form is required for each prospective advisory committee member.  If no advisory member prospects (or new advisory committee members) need to be considered, these forms DO NOT need to be provided.  Churches open less than one year typically do not have advisory committee prospects to present yet.

ChurchOfficerRecommendations.doc - This form MUST be completed and turned in by every pastor, even if there are no advisory committee members and thus, no named officers yet.  In such cases, the pastors name is the only one that needs to be recorded prior to giving the form to the area presbyter for review.  A completed document signed by the pastor and area presbyter should be presented by the due date of September 9, 2011.

Balance.Sheet.doc - This form should be completed using the worksheet provided (see BalanceSheet_Worksheet.doc), or an equivalent balance sheet can be submitted in its place assuming that all the information is complete and accurate.  The worksheet DOES NOT need to be submitted along with the balance sheet for the annual review.

2011Income-ExpenseStatement.doc - This form should be completed or an equivalent income and expense statement can be submitted in its place assuming that all the information is complete and accurate.  The budgeted and actual numbers should be provided for the first eight months of the year.

2012 Proposed Budget.doc - This form should be completed using the worksheet provided (see Budget.Worksheet.doc), or an equivalent budget can be submitted in its place assuming that all the information is complete and accurate.  The worksheet DOES NOT need to be submitted along with the proposed budget for the annual review.

HousingElectionForm - One form should be completed and submitted for the pastor and each compensated member of the church's ministerial staff for whom a housing allowance is to be provided.

BAP.ElectionForm.doc - One form should be completed and submitted for the pastor and each compensated member of the church's ministerial staff for whom an accountable business and professional expense account is to be maintained by the church.

Cafeteria.Plan.doc - One form should be completed and submitted for the pastor and each compensated member of the church's staff for whom a cafeteria plan (medical expenses) is to be administered by the church.

Balance.Sheet_.Worksheet.doc - This worksheet is an aid to help in the completion of the balance sheet and DOES NOT need to be submitted with the annual review documents.

Budget.Worksheet.doc - This worksheet is an aid to help in the completion of the proposed budget and DOES NOT need to be submitted with the annual review documents.

Dues.Worksheet.doc - This worksheet is an aid to help in the determination of ministerial dues and DOES NOT need to be submitted with the annual review documents.
